Failure questions are answered badly in two ways: a disguised success, or a failure with the blame placed elsewhere. Strong answers name the miss plainly, take the share of responsibility that is genuinely theirs, and are specific about the mechanism — what they misjudged, when they knew, and what they did the moment they knew. Communication is usually the real test: telling the stakeholder early is the behaviour the interviewer is looking for. The reflection should name a change in practice that a listener could verify.
